The Clinical Program Manager is responsible for improving the quality, efficiency and effectiveness of nursing care provided to patients of a specialty care clinical program (e.g., geriatrics, enterostomal therapy) by conducting ongoing needs assessments to determine patient care improvement opportunities. This job also plans and provides nursing education programs to improve staff capabilities in providing care to these patients.

In this role you will be:




Responsible for overall quality and data management for the Structural Heart program at VMMC. Tracks data for the National Cardiovascular Data Registry (NCDR®) for TVT. The TVT Registry has been approved by the Centers for Medicare and Medicaid Services (CMS) to meet the registry requirements outlined in the national coverage decisions for transcatheter aortic valve replacement (TAVR) and transcatheter mitral valve repair (TMVR).
Collaborates with multi-disciplinary team on program design, planning, and improvement work. Multi-disciplinary interactions include, Cardiology, CT Surgery, Cardiac Anesthesia, Cath Lab, OR staff, Inpatient Staff, etc.
Facilitates communication to the multi-disciplinary team about upcoming procedures, plans of care, and pathways for SH patients.
Stays up to date with changes in practice, informs medical and administrative leadership and advocates for resources.
Sees all SH patients pre-procedure to provide education, what to expect before, during, and after, and providing instructions for home care and follow up. Directly involved with providers on patient workup and developing plan of care for the procedure. Coordinates patients’ optimization, imaging needs, etc. pre-procedure.
Consults with and directs support staff on each patients’ plan of care post Valve conference.
Educates patients and caregivers prior to and post procedure.
Educates support staff and non-structural providers about Structural Heart program, when/who to refer, benefits of the program, etc.
At least twice annually provides updates on overall program performance to quality committee and reports out in other venues as appropriate. Provides in-service education to Cath lab, inpatient, and ambulatory nursing staff for pathway patient management practices.
Helps to identify programmatic capital and staffing needs.

BSN, and current, valid Washington State Registered Nurse License, required.
Relevant clinical skill/experience also required.
Ability to analyze complex pathophysiological and patient care problems.
Experience in program planning, implementation and evaluation.
The ability to identify the educational needs of the patients, family, and staff, and to develop and implement appropriate educational programs.
Public speaking skills.
Budgeting experience
Excellent organizational, interpersonal and communication skills.
Specialty certification may be required.
Master's degree in Nursing preferred.
