The
Post-Acute Program Manager
at the
University of Maryland Rehabilitation & Orthopaedic
Institute
(UM Rehab & Ortho) serves as a liaison between University of Maryland Rehabilitation & Orthopaedic Institute (UM REHAB) leadership, and long-term care partners leadership to oversee program implementation to reduce readmissions and provide value-based care across the continuum. The program manager is responsible for managing the program on a day to day basis, effectively communicating program expectations to stakeholders and leadership. Responsibilities include organizing and managing teams, developing project plans and schedules, managing issues, risks and communicating at all levels of the organization. Ideal candidates should have knowledge and experience in care coordination and transitions of care management, as well as experience in monitoring outcome measures using analytical tools.

What You Need to Be Successful:
Education and Experience
Bachelors of Science in Nursing required
At least 10 years' experience in Healthcare including 3-5 years of related experience managing care coordination programs.
Knowledge, Skills and Abilities
Assigning work priorities, auditing work quality, and orienting and training new staff.
Ability to work in partnership with post-acute care providers to building relationships and change behavior.
Knowledge of both the clinical and operation of various post-acute care provider including payment systems as well as assessment tools.
Knowledge of strategic planning and program development activities as related to patient flow management.
Knowledge of budget development, control, and evaluation is required.
Knowledge of risk sharing payment models and the ability to develop a business plan and preform around the model.
Knowledge of the operation of personal computer and information systems is required.
Proficient skills in directing, monitoring, evaluating and motivating the performance of staff.
High level of proficiency and demonstrated effectiveness in problem solving and implementing new programs related to increased departmental and organizational efficiency.
Highly effective written and oral communication skills are required to work with all levels of staff within UM REHAB organization as well as our post-acute care partners.
Highly effective organizational and interpersonal skills.
Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served. Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient's requirements relative to his or her population-specific and age specific needs.
