MARY AND ELIZABETH HOSP.','','!*!The Program Manager, Patient Safety coordinates the patient safety program within a facility-based
setting and serves as an advocate for patient safety by assessing, developing, assisting with
implementation, evaluating and providing oversight for strategies, programs and services that reduce
patient risk and optimize patient safety. The Program Manager, Patient Safety is responsible for
supporting day to day continuous patient safety improvement to reduce healthcare errors and healthcare
incidents. The role functions as a leader in promoting systems that foster the development of a culture of
proactive patient safety.
The Program Manager, Patient Safety has an integral role in maintaining the direction of strategies for
the facility-based patient safety program and developing a culture of safety in the workplace. The
Program Manager works in collaboration with employee and environmental safety. The Program Manager
develops the core components of the Healthcare Performance Improvement (HPI) Safety First Model
and in partnership with the Quality, Risk Management and Operational leaders, ensures an environment
that encourages communication of safety concerns and unsafe conditions and assumes responsibility for
the implementation and subsequent monitoring of results. The Program Manager is the facilitator for
Safety First Initiatives and responsible for engagement with Safety First Error Prevention Techniques.
The Program Manager, Patient Safety identifies methods for reduction of healthcare errors and other
factors that contribute to unintended adverse outcomes. The Program Manager communicates literaturebased
ideas regarding effective patient safety strategies to others within the hospital and facilitates
implementation of the patient safety program within the hospital.
The Program Manager, Patient Safety utilizes data to proactively identify and address any patient safety
issues to keep patients protected from unintended harm. The Program Manager participates in
processing and documenting patient safety events and ensures all activities are done in accordance with
patient safety, regulatory, accrediting and local, state and national guidelines as applicable. The Program
Manager, Patient Safety assists with performing clinical investigations, reviews near misses and
communicates trends and patterns that will facilitate improvement.Essential Functions:General Patient Safety Program Support:-Facilitate and support Patient Safety and the SafetyFirst Culture Transformation Efforts.-Promptly notify key administrative and leadership stakeholders when an event is identified.-Provide leadership to facility and unit-based safety huddles.-Display
knowledge and comply with all National Patient Safety Goals, Patient
Safety System Standards and other regulatory agencies.-Instruct clinical and ancillary colleagues regarding the SafetyFirst Error Prevention Techniques.-Proactively facilitate and lead efforts to design system initiatives that will minimize the likelihood of future errors.-Partner
with Quality and Risk Management to facilitate the Serious Safety Event
(SSE) root cause analysis (RCA), event investigation and ensure the
completion of associated reports and documents.-Partner with Quality
and Risk Management to perform failure mode effects analyses (FMEA) and
identifying high risk processes and systems to proactively promote
safety and error prevention.-Incorporate human factors analysis into the review process as appropriate.Partner
with Risk Management to participate in the review and analysis of
occurrence reports, causal factors and determine action plans needed to
prevent error.-Provide leadership in Apparent Cause Analyses on precursor and potential near miss events.-Provide
support for the event reporting workflow to ensure appropriate, timely
identification of events requiring root cause identification and
analyses, and related performance improvement measures and project
timelines.-Partner with Risk Management and Legal in establishing
and maintaining processes to ensure timely event review to ensure
relevant event data submission to the patient safety organization.-Develop relationships with medical staff, clinical staff and leadership to facilitate SafetyFirst Culture Transformation.-Assist with the deployment of the AHRQ Culture of Safety Survey and work with leaders on reporting and action plans.-Perform
periodic safety “rounds” to proactively identify safety risks and
hazards and recommend enhancements for improving patient, employee and
environmental safety.-Assist with implementation of reliable systems to support evidence-based care to improve clinical outcomes and patient safety.-Promote transparency of Patient Safety data and stories.-Committee and Performance Improvement Support:-Active
committee member for incident review and classification and assist with
identifying each incident as a near miss, precursor event, or serious
safety event.-Serve on committees related to quality, patient
safety, clinical effectiveness, medication safety,and/or patient
experience as requested.-Support leadership and staff with the development and implementation of patient safety process changes.-Participate in unit-based process change activities and evaluate the effectiveness of process changeinitiatives.-Assess for safety improvement opportunities and provide feedback to support these patient safety improvement activities-Utilize performance improvement tools for process improvement.Data Management, Analysis & Reporting:-Analyze multiple data and information sources to identify safety initiatives.-Establish an auditing process to assess compliance with safety structures, processes and measures.-Review
internal safety reports and utilize information from external reporting
programs to mitigate risk (ie: Institute for Safe Medication Practices
(ISMP) Medication safety alerts, Emergency Care-Research Institute (ECRI), Joint Commission Sentinel events).-Provide trend reports to departments.-Measure,
analyze, develop and present reports; offering support to departmental
initiatives and quality/safety improvement teams.-Analyze and
present adverse trends for review by appropriate committees and assist
in development of action plans for resolution.-Ability to identify
and define problems, collect data, establish facts and draw valid
conclusions around patient safety initiatives by means of measurable
results.-Participate in and coordinate comprehensive facility data
collection pertaining to patient safety to meet national, state,
regional and internal measures.-Collect, analyze and effectively display / present data and promote a collaborative approach toward goal achievement.-Collect accurate quantitative and qualitative incident data from appropriate resources.Education, Training and Consultation:-Communicate to facility stakeholders, the Quality, Risk and Safety alerts outlining lessons learned.-Design and/or deliver educational presentations that facilitate the understanding and implementation of SafetyFirst principles.-Oversee,
develop and participate in Patient Safety orientation, onboarding and
educational endeavors, annual recurrent training, one-on-one
consultations and in-service training to clinical and non-clinical
staff.-Conduct regular patient safety walk-rounds / safety rounds to
provide „just in time‟ education and Provide leadership, coaching and
mentoring to Safety -Coaches on the frontline who coach and mentor
healthcare team members on the consistent use of the safety behaviors
and techniques.-Lead Human Error Prevention Training and Safety
First Team Training to improve communication and teamwork skills for all
members of the healthcare team.-Disseminate patient safety best
practices and educational materials and communicate literature based
ideas regarding effective patient safety strategies to others within the
organization-Provide mentorship and act as a resource to other quality, risk management and patient safety staff.','!*!Education / Accreditation / Licensure (required & preferred):Bachelor‟s degree in healthcare related field preferred.
